• Birth Certificate or Marriage Contract (Photocopy)
• Diploma / TOR / High School Card / K to 12 certificate of Completion / ALS Certificate of grades
• Medical Certificate (original copy)
• Barangay Clearance (original copy)
• 4 pcs 1x1 ID pic (white background with collar)
• 4 pcs passport size ID pic (white background with collar and Computerized name tag) note: Digital photo printing only
• Accomplished learners profile form
